MENTAL HEALTH SUPPORT IN HEREFORDSHIRE

SureCare Hereford has been selected to work in partnership with Herefordshire Council and Herefordshire Mental Health Services on a pilot scheme to provide a specialist intermediate care service in the county.
The service will enable individuals with mental heath needs, to remain in the familiar and secure environment of their own homes and reduce the need for hospital or residential admission. The service is available on discharge from hospital or in the absence due to illness, of a personal carer. Support is available from a physiotherapist, occupational therapist, specialist community nurse and the services of the SureCare specialist support workers team. The team will provide intensive support to service users to gain confidence and independence in personal care, general household tasks and encourage involvement in the local community. Angela Gilchrist, SureCare's Operations Manager said: "Our care support staff are highly trained in specialist care and our excellent reputation in Hereford has resulted in us being successful in this latest contract bid." The specialist service is initially offered for six weeks after which, ongoing care is often arranged. SureCare Hereford have 62 trained carers working in the county, providing domiciliary, mental health and palliative care to people in their own homes.
